One thorny example that cropped up recently is the tag 'eswc2006' on Flickr [1]. It's been used to tag photos from this years European Semantic Web Conference (in Budva, Montenegro), and from the 2006 Electronic Sports World Cup (Paris, France). The eventy/conferencey nature of both these things makes it a pretty hard one to disambiguate, even for humans. The pictures of the Eiffel Tower [2] are the only real clue that the photos are of two different events, but even this requires a lot of background knowledge, e.g: "the eiffel tower is in paris, paris is not by the sea".
